Care Coordinator

Minimum Requirements:

  • Bachelor’s Degree in Social Work from an accredited school of social work (required)
  • Basic social worker skills including but not limited to empathy, responsiveness to cultural and ethnic diversity, advocacy, communication, and crisis intervention (required)
  • Knowledgeable about internal, community and care resources (required)
  • Motivational to encourage clients to follow their care plans
  • Strong verbal and communication skills to explain to clients, family members, and professionals the case and care plan and maintain good records
  • Organization to maintain several different cases at once

Program Coordinator

Minimum Requirements:

  • High School Diploma or equivalent
  • At least 1 year of healthcare experience
  • Proficient with Microsoft Office products (Word, Excel, PowerPoint etc.)
  • Data entry experience
